The door edge SUCKS and kills my arm. I can't break myself of trying to use it either! It would be one thing if these were manual transmission, but not only was there no consideration for it, it's almost as if they made it painful on purpose.
I'm going to really reach here.. maybe the outward sloping angle helps deflect rain while at Starbucks? Seriously though, why mak it a crowned edge?
Has anyone found a solution?

It's actually pretty nice for me. I never put my arm there if the window's up, and it's the right angle for me if the window's open, better than a flat (horizontal) surface.

I’ve had the same issue with the GX. PrimoWorkz has the solution! I had to slightly bend the plate to get it to fit perfectly.

I suppose this is purely influenced by height, arm length, and chair position.. I'm only 5'9", but I have a longer torso and 5'11" wingspan. I adjust my seat in a position that allows my wife to not need to readjust the rearview mirror when she drives.
 
I agree that the crowned, hard plastic door sill is awful.

I tried the Primo workz thing on a past truck and I had to return it - it was just too big & bulky and ugly for me.
 
This is clearly a driver size, seat position and what-you-do-with-your-arm issue.

I would never rest my arm there with the window up. And with the window down and my elbow sticking out, the angled surface is perfect for me to rest my arm - my arm's at a roughly 30 degree (or whatever) downward angle and so is the door top surface. If it was flat I'd be resting my arm on only the outside edge and / or the window gasket. It's perfect as-is imho. It's actually one of the unexpected cool things about the truck for me.

So I've never had a problem with the door/arm rest. Now that I'm reading it I'm sure there is some kind of forum-phobia that will require changing out all the doors, redesigning/fabricating new pieces, or even worse - someone posts a web site with a 'new improved' GX door rest that I must buy now for a mere $7500. Thanks everyone!!
 
Bringing this thread back to life a little bit. The hard pointy plastic on the door continues to bother me, and now I also realize that the top half of the door card is probably the hardest plastic I have ever seen in a modern toyota product.

LC250s and 5th gen 4runners both have softer, plusher material making up the top half of the door card. These door cards totally need a redesign. This is not befitting a "luxury" vehicle.
